The game is set in a low-fantasy realm containing a diverse array of humanoid races, including humans, goblins, elves, dwarves, halflings, giants, ogres, vampires, and bat-like creatures. The setting is a feudal society governed by a Baron, with the town of Weirdwood and the city of Greyport as key population centers. A prominent religious order serves the Lord of Light, while the goddess of sex and fertility, Qetesh, and the underworld god Nergal maintain active influence. Scattered ruins of an advanced civilization known as the Ancients contain powerful artifacts and portals.
The world operates under a physics of magic, with Waystones enabling fast travel between locations, and crystals capable of focusing magical energy. Unique mechanics exist, such as the crystallized eggs used for breeding and reproduction, fertility and enhancement potions altering biological functions, and a "Spirit World" accessible through potions to relive past events. Various monsters like dire wolves, grumpkins, werewolves, and dragons inhabit the land. Social norms permit polygamous relationships, and the practice of capturing and selling creatures and slaves is common.

The protagonist is summoned to the village of Weirdwood and immediately gets entangled in its problems, helping the locals with goblin infestations, bandit raids, and missing persons. He meets the priestess Ziva, who tasks him with restoring a temple of Qetesh, a key objective that introduces him to other women who will join the temple and become potential mates. The central conflict expands with the emergence of a militant order of Purifiers who seek to destroy the portals connected to the supernatural realm. A significant portion of the plot is devoted to the protagonist's relationships, particularly with Erevi, a dark elf sorceress. The overarching narrative focuses on the growing threat of Nergal, the god of the underworld, who seeks to enter the mortal realm. The protagonist and his female allies must defend against Nergal's minions, culminating in a battle where he learns he is the "chosen one" destined to father many children to empower Qetesh and defeat Nergal. Upon securing a marriage with Erevi and uniting the various factions, the protagonist defeats Nergal and assumes the role of Lord Protector, dedicating his life to protecting his growing family and the realm.
The central narrative path focuses on the high elf Erevi, who is a sorceress and priestess. She helps the protagonist understand his role in the conflict with Nergal. Alice is the innkeeper's daughter who is trapped by her father's strict control. Her route centers on her quest for freedom and a life away from her family.
