The setting is Grimhaven, a city in Washington State, during a severe biological outbreak caused by an extraterrestrial artifact known as the Artifact. The contagion, a mutative agent, primarily spreads through bodily fluids and transforms humans into infected beings driven by an overwhelming, primal urge for reproduction. This results in public, compulsive sexual acts. The infection induces a euphoric, aphrodisiac-like state in victims, suppressing resistance and promoting the contagion's spread. The Artifact, housed at Faraday Pharmaceuticals Research Facility, is the source, and the company's secretive research exploits the event for biological advancement, viewing the city as a contained test site. The physical world becomes a maze of destroyed urban infrastructure, burning buildings, and high concentrations of the infected, where stealth and improvisation are key to survival. The mutated contagion creates evolved lifeforms, including tentacled nest-builders and powerful predator variants like the Plague Husk and werewolves, and can influence animal life. A secret service subway system runs beneath the city, controlled by Faraday, indicating a prepared infrastructure for covert operations.

The narrative begins with Sergeant Sarah Hayes and her younger sister Ava in their apartment as the outbreak commences. They are forced to flee into the chaotic city streets after an explosion blocks their escape route. During their first encounter with the infected, Sarah and Ava are separated, leading to Ava being assaulted and infected by a corruptor monster in a warehouse. The sisters reunite but constantly face new threats, including mutated creatures and hostile groups of survivors who exploit others. They seek refuge at a police station, which becomes overrun by the infected. Their eventual destination is Faraday Pharmaceuticals, the only place Sarah believes can help Ava. Upon arrival, Sarah is taken into custody, and it is revealed that she is immune to the contagion, while Ava is infected and resistant. The facility director, Kato, and lead scientist, Rakita, inform Sarah that she must venture back into the city to gather samples of the evolving contagion to create a cure for Ava. Sarah agrees, setting off on a solo mission. Her journey involves multiple locations: a forest cabin, a mall, and later, deeper city areas. Meanwhile, Ava is used as a test subject within Faraday, being forced to engage with infected specimens to generate new viral strains for research.
The younger sister of Sarah Hayes. She is a civilian with a rebellious and somewhat sarcastic nature, now grappling with a severe infection that threatens to consume her identity. Her survival and her bond with Sarah are her primary driving forces.
Pink hair tied back, pale skin, and a lean body. She is often depicted in casual, functional clothing suited for survival, like hoodies and jeans. As her infection progresses, her breasts swell, and her womb bears a pulsing mark of corruption, indicating her pregnancy with monstrous eggs.
Initially impulsive and reliant on her sister for protection, Ava is transformed by the outbreak. She exhibits deep insecurity, fear of her own desires, and a profound internal conflict. Her core flaw is her vulnerability to the contagion's influence, which weaponizes her deepest psychological needs against her. She copes through denial, sarcasm, and moments of desperate resistance, but also succumbs to pleasurable desires.
The traumatic loss of her parents, forcing her to depend solely on her sister Sarah for support and stability. Her initial infection and assault by the corruptor in the warehouse, which plants the seed of the contagion within her and begins her transformation. [Core Bond] Her deep, loving, and protective bond with her sister Sarah, which fuels her will to resist the corruption and serves as her primary emotional anchor throughout the ordeal.
