The island of Sylphir is a windswept, magical land accessible only by airship, distinct from the mainland’s feudal politics. Its capital, Aerilon, is a bustling hub of merchants, fortune-tellers, and adventurers. The island features diverse biomes: the Vinara Vineyards on Whispering Beach, the cursed Forbidden Wood (Blackwood) containing the hidden Forbidden Library of the Occult, the ruined town of Porzella—frozen in stone by a medusa—and the remote Ashen Isle with Onyxia's White Tower. A new cosmic threat emerges in the form of Crimson Stalkers—floating, demonic eyes that speak Infernal Low and scout for the demon lord Baalorix Fleshreaver’s lost daughter. The island’s magical ecosystem includes ancient relics, dimensional rifts, and a secret portal to the infernal realm.

The DLC begins when the pilot Sera offers Horos a flight to Sylphir aboard her airship Aurora Borealis. Upon arrival, Horos witnesses a Crimson Stalker—a floating demonic eye—babbling in an unknown tongue. Bibi, a local brothel madam, hires him to investigate their purpose. Sera directs him to the fortune-teller Celestia Voss, who demands 500 gold and sand from Whispering Beach for a ritual. On the beach, Horos saves vintner Flavia from her jealous sea-creature lover, Lirion. Celestia’s vision reveals a rising darkness. Following leads, Horos enters Blackwood to find the Forbidden Library, guarded by the virgin librarian Selena. To gain entry, he must learn a password from a tiny dark fairy, who demands legendary grapes from Flavia’s vineyard. Inside, Selena refuses help until Horos brings her Frigi’s journal (from Celestia), proving that the Dark Lady Nyxara is dead and Selena’s vigil is over. Freed, Selena performs a ritual granting Horos the Arcane: Demon Language—consuming his own semen from a chalice to bind the infernal tongue. Now able to speak to the Stalkers, Horos learns they seek Lilithra Vexia, Baalorix’s daughter. Bibi confesses she is Lilithra, a succubus who fled the infernal realm. To close the dimensional rift she created, Horos obtains a scroll of Dimensional Severance from the petrified mage Eldric’s tower in Porzella. However, the rift is held open from the infernal side by Carnifex, Baalorix’s enforcer. Horos travels to the Ashen Isle to meet the exiled demon-hunter Onyxia, who forges a star-iron dagger capable of harming demons. He ventures into the infernal realm, kills a Voracita, and brings its horn to Onyxia. He then negotiates with Sanguira Vex, Bibi’s demonic sister, who agrees to help frame Carnifex for Lilithra’s murder. The plan requires fake evidence: Bibi’s hair, clothing, and a demonic pendant (recovered from a bandit cave). To seal the deal, Sanguira demands the erotic book The Goat and the Village Girls (obtained from Selena). The framing succeeds: a witness (Oog) confesses under duress in the Hall of Gluttony, and Carnifex is branded a traitor. Enraged, Carnifex attempts to pierce the Heart of Baalorix to seize power. Horos confronts him in the Heart Chamber, kills him, and closes the rift, securing Bibi’s freedom. The DLC ends with two major branching endings: a redemptive celebration with all allies, or a dark betrayal where Horos enslaves the women of Sylphir for Baalorix.
Horos accepts Carnifex’s offer and betrays Sylphir. He hands over Bibi and enslaves Celestia, Dhalia, Onyxia, Flavia, Selena, and Sera. He sexually humiliates each woman—fucking them on thrones, covering them in cum, and asserting dominance. After the conquest, he sits alone on a throne of bones, realizing the emptiness of his power. The ending is bleak: Horos is a lonely tyrant, rejected by all, with no genuine connection or happiness.
Horos refuses Carnifex’s offer and completes the framing of the demon. Bibi and Sanguira reconcile and open a brothel together in Aerilon. The townspeople celebrate Horos as a hero. In a final scene, Horos has a threesome with Bibi and Sanguira—anal, vaginal, and oral sex, with cumshots on faces and bodies. The sisters thank him for giving them freedom and a new life. The ending is warm, communal, and sexually satisfying, with the promise of future visits and a found family.
